  • Study compares HCC patients with and without MASLD after surgical resection.
  • 906 treatment-naïve HCC patients undergoing curative resection (2016-2022) were analyzed.
  • Patients categorized as non-MASLD (n=496), MASLD without MASH (n=337), or MASH (n=73).
  • MASLD patients had higher BMI, more cardiometabolic risk factors, and earlier tumor stages.
  • 5-year overall survival (OS) was better in MASLD patients (76.9%) vs. non-MASLD (71.1%).
  • MASLD without MASH had the highest 5-year OS (79.6%), while MASH subgroup was comparable to non-MASLD (64.7%).
  • Simple steatosis (MASLD without MASH) showed improved OS, whereas MASH predicted poorer OS.
  • Cardiometabolic risk factors alone did not affect OS.
  • Study highlights the need for histological differentiation in postoperative management of HCC patients.
