  • Nearly half (47%) of heart failure with reduced ejection fraction (HFrEF) patients received potentially inappropriate prescribing (PIP) medications.
  • Common PIP medications included neuroleptics (29%), systemic steroids (10%), and NSAIDs (6%).
  • Independent predictors of PIP included rheumatoid arthritis, depression, chronic obstructive pulmonary disease (COPD), and gout.
  • Patients on PIP were less likely to receive guideline-directed medical therapies (GDMT).
  • PIP was associated with increased risks of HF death, all-cause and cardiovascular death, and HF hospitalizations.
  • The study highlights the need for strategies to reduce inappropriate prescribing in HFrEF patients.
