  • Gamma Knife Radiosurgery (GKRS) is an effective treatment for medically intractable trigeminal neuralgia (TN).
  • The study evaluated whether neurovascular conflict (NVC) influences pain and sensory outcomes post-GKRS.
  • 148 patients who underwent primary GKRS for TN between 2014-2024 were retrospectively reviewed.
  • 89% of patients achieved significant pain relief (BNI grade IIIb or better).
  • Pain recurrence occurred in 20% of patients, and 32% developed facial sensory disturbances.
  • Estimated pain relief rates were 77% at 1 year, decreasing to 64% at 7 years.
  • NVC presence/severity did not predict pain or sensory outcomes post-GKRS.
  • Facial sensory disturbances were associated with long-term pain relief.
