  • Osteoporosis is a systemic bone disease characterized by excessive osteoclast differentiation, leading to reduced bone mass and increased fragility.

  • The hydrogel releases NaHCO3@NPVs at the defect site, which are internalized by bone marrow-derived macrophages (BMDMs).
  • NaHCO3@NPVs inhibit osteoclast differentiation and activity through a 'biochemical dual-action' mechanism: alkalinizing lysosomes and depleting lysosomal calcium, and bioactive factors in NPVs directly or indirectly inhibiting osteoclasts.
  • In vivo experiments in a rat model showed that NaHCO3@NPVs-EH reduced overactive osteoclasts and enhanced bone repair.
  • The study suggests NaHCO3@NPVs-EH as a promising treatment for osteoporotic bone defects and introduces a new acid-base neutralization perspective for osteoporosis treatment.
