- The R2D2-ICU trial compared restrictive vs liberal wrist-strap physical restraint strategies in critically ill patients on mechanical ventilation.
- 405 adult patients across 10 French ICUs were randomized to either a restrictive (low-use) or liberal (high-use) restraint strategy.
- Primary outcome: No significant difference in days alive without coma or delirium at 14 days between groups (6.67 vs 6.30 days, P=0.51).
- Secondary outcomes: Similar rates of self-extubation (9.2% vs 8.5%) and day-90 mortality (37.2% vs 41.0%) between strategies.
- Conclusion: Low-use restraint strategy did not improve delirium/coma-free days compared to high-use strategy in ventilated ICU patients.
