  • A parsimonious echocardiography-based model was developed to predict long-term all-cause mortality in heart failure with preserved ejection fraction (HFpEF).
  • The model was derived using LASSO-penalized Cox regression and included age, left ventricular posterior wall thickness at end-systole, mitral E velocity, E/e' ratio, and pulmonary artery systolic pressure.
  • The study involved 792 adults with HFpEF, split into training (n=554) and validation (n=238) cohorts, with median follow-up periods of 5.17 and 5.75 years, respectively.
  • The nomogram showed better discrimination for mortality prediction compared to HFA-PEFF and H2FPEF scores, with higher area under the curves at 1, 3, and 5 years.
  • Calibration was acceptable at 1 and 3 years but weaker at 5 years, and risk-score tertiles effectively separated survival in both cohorts.
  • External validation is needed before clinical implementation of the model.
