  • Primary CNS T-cell lymphoma (PCNSTL) is a rare and aggressive non-Hodgkin lymphoma with nonspecific clinical presentations.
  • The study analyzed 16 patients (median age 50, 11 male) with histologically confirmed PCNSTL from 2008 to 2024.
  • MRI scans showed supratentorial parenchymal lesions with contrast enhancement in 87.5% of cases, and multifocal lesions in 43.75%.
  • PET imaging revealed low to moderate tracer uptake in 87.5% of examined patients.
  • Median progression-free survival was 4 months, and median overall survival was 97.5 months.
  • Methotrexate-based chemotherapy combined with autologous stem-cell transplant (ASCT) was associated with the most favorable outcome.
  • An ALK1-positive PCNSTL case achieved complete remission after treatment with the ALK-inhibitor lorlatinib.
  • The study suggests distinct neuroimaging patterns may help in identifying PCNSTL cases in the future.
