- Sepsis is a leading cause of ICU mortality, often complicated by secondary infections due to immunosuppression.
- ICU-acquired infection (IAI) increases the risk of chronic critical illness (CCI), but there are few established biomarkers for IAI.
- This study analyzed data from the MIMIC-IV database (17,209 patients) to explore the association between early blood lactate levels and IAI risk.
- A linear dose-response relationship was found between admission lactate levels and IAI risk.
- Hyperlactatemia (>6 mmol/L) was independently associated with increased odds of IAI (OR = 1.36; 95% CI, 1.08-1.70).
- Elevated lactate predicted higher 28-day ICU mortality in patients without IAI, but not in the IAI cohort after adjustment.
