  • Bronchoalveolar lavage (BAL) lymphocytosis and eosinophilia are associated with immune checkpoint inhibitor pneumonitis (ICIP).
  • The study evaluated BAL immune cell percentages (lymphocytes, eosinophils, neutrophils) as diagnostic biomarkers for ICIP.
  • Higher lymphocyte and eosinophil percentages were independently associated with ICIP, while neutrophil percentage was inversely related.
  • The diagnostic performance of BAL lymphocytes (AUROC 0.62) and eosinophils (AUROC 0.61) for ICIP was modest.
  • BAL immune cell percentages also showed associations with 1-year survival in patients with pneumonitis.
  • The study suggests BAL immune cell percentages may have prognostic value but limited diagnostic accuracy for ICIP.
