Diagnostic Value of the Kappa Free Light Chain Index to Distinguish MOGAD, NMOSD, and MS - PubMed

- The Kappa Free Light Chain Index (KFLC-Index) is a biomarker for distinguishing multiple sclerosis (MS), aquaporin-4-immunoglobulin G-positive neuromyelitis optica spectrum disorder (AQP4-NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD).
- A retrospective study of 303 patients found median KFLC-Index lowest in MOGAD (0.60), intermediate in NMOSD (8.85), and highest in MS (49.90), showing very good diagnostic performance for differentiating MOGAD from MS (AUC 0.93).
- KFLC-Index had good performance in differentiating NMOSD from MS (AUC 0.82) and MOGAD from NMOSD (AUC 0.77), and combining it with CSF oligoclonal bands (OCB) improved diagnostic accuracy over either test alone.
- The study suggests KFLC-Index as an additional supporting criterion for differential diagnosis between MOGAD and MS, providing complementary information to OCB.
